Contracting is a foundational model in TA - if you have no contract, you are not doing TA. Whenever there is a problem, it is usually due to a poor contract. People do not always say what they mean, they feel intimidated or maybe they can't be bothered to be clear and give exact information. Somewhere down the line, things go wrong: the materials do not arrive; the people are confused about the timing; maybe some transference is going on and they felt too scared to say what they needed to say.
So problems arise even when there are only two parties involved. In organisations, the number of teams, individuals, specialists, suppliers etc involved add a complexity where a lack of clear communication can have catastrophic results.
